How Cosmelan works

Cosmelan is a two-part professional depigmentation system developed by Mesoestetic. Cosmelan 1 is the in-clinic mask, applied in a thick even layer across the whole face and worn home for eight to twelve hours before being washed off. Cosmelan 2 is the at-home maintenance cream, started on day two and used twice daily, then tapered down across a structured six-month regimen with scheduled clinic reviews along the way.

The active ingredients target the enzyme tyrosinase, which controls melanin production in the skin. By suppressing tyrosinase activity over weeks and months, Cosmelan reduces the production of new pigment while the existing pigment in the upper skin layers sheds naturally. The depth and the structure of the protocol are what make it effective on stubborn melasma where surface peels and topical creams alone fall short.

It is also one of the few professional depigmentation protocols safely used across the full Fitzpatrick scale, from one through to six, which is why it is so widely chosen for higher-melanin skin types where the risk of post-inflammatory hyperpigmentation rules out many lasers and stronger peels.

What the six-month regimen looks like

Day zero is the in-clinic mask appointment. Allow seventy-five minutes. The skin is cleansed and prepared, the Cosmelan 1 mask is applied across the whole face, and you go home wearing it. You wash it off at home after the agreed time, usually eight to twelve hours, following the written protocol. You start the supporting moisturiser the same evening.

Day two onwards is the home phase. Cosmelan 2 cream is used twice daily, layered under SPF 50 in the morning and used on its own at night. The first three to four weeks involve significant flaking and renewal as the upper layers of pigmented skin shed and fresher skin emerges underneath. Patients typically see the first visible lightening between weeks four and eight.

From there the cream is tapered down based on how your skin is responding, with scheduled clinic reviews at the agreed milestones. The full regimen runs for six months. Long-term maintenance, including strict daily SPF 50 and avoiding strong sun, is essential to hold the result, particularly for hormone-driven melasma which is prone to coming back without it.

It is not the right call for patients who cannot commit to the strict daily SPF 50 and the structured six-month home regimen, because the result depends on the discipline as much as on the in-clinic mask. It is also not appropriate during pregnancy or breastfeeding, with isotretinoin in the last six months, with an active skin infection or eczema flare on the face, or on tanned or recently sun-exposed skin.

Allow about seventy-five minutes for the mask application. The skin is cleansed and prepared in clinic, then the Cosmelan 1 mask is applied in a thick, even layer across the whole face. You wear the mask home, with clear written instructions on how long to keep it on, which is usually eight to twelve hours depending on your skin type. You wash the mask off at home following the protocol, then start the supporting moisturiser. Cosmelan 2 cream begins on day two and is used twice daily. What downtime and side effects should I expect? +

The first three to four days after the mask are the most active. The skin feels tight, looks pink or red, and starts to flake significantly between days three and ten. Most patients arrange for the first week to be a quieter social week or take a few days at home. Light cover-up makeup is fine from day four or five for most patients. Do not pick, peel or exfoliate the flaking skin manually; let it shed naturally or you risk a pigment patch where the skin was disturbed. Strict daily SPF 50 is non-negotiable throughout the six-month regimen because the skin is more photosensitive while it remodels.

When will I see results and how long do they last? +

We do not treat patients who are pregnant or breastfeeding, who have an active skin infection or eczema flare in the treatment area, who are on isotretinoin currently or in the last six months, who have a known sensitivity to any component of the Cosmelan formulation, who have a tan or recent strong sun exposure on the face, or who have a history of keloid scarring on the face. We also do not start the protocol with patients who cannot commit to the strict daily SPF and the structured six-month home regimen, because the result depends on the discipline as much as the mask.
