Do B12 injections need to be refrigerated?

The short answer is that it depends on which form of B12 you have. The two used for injections, hydroxocobalamin and cyanocobalamin, have different storage rules, and getting them mixed up is the usual source of confusion. The label on your vial is always the final word, but here is the general picture and why it matters.

Hydroxocobalamin: keep it refrigerated

Hydroxocobalamin is the form the NHS uses for treating B12 deficiency, because it stays in the body longer. It is generally stored refrigerated at 2 to 8C and protected from light. Cyanocobalamin: usually room temperature

Cyanocobalamin is generally stable at controlled room temperature, away from heat and direct sunlight, though you should still check the BNF entry and the product leaflet for the exact storage line. Different manufacturers can specify slightly different conditions.

Why storage matters

Three things are at stake when B12 is stored correctly:

  • Potency. Heat and light can degrade the vitamin, so a poorly stored vial may be weaker than it should be.
  • Sterility. Injectable medicines must stay sterile, which is part of why they are kept in controlled conditions and never decanted or topped up.
  • Shelf life. Correct storage keeps the product usable up to its printed expiry date.

Home storage basics

If you are keeping B12 at home on a clinician’s advice:

  • Store it on a middle fridge shelf, not in the door, where the temperature swings most.
  • Keep it in the original carton to protect it from light.
  • Never freeze it. Freezing damages B12, and a frozen vial should be discarded.
  • Keep it separate from food, and note the date you first opened any multi-dose vial.

What spoiled B12 looks like

A vial that is cloudy, discoloured, has visible particles or an unusual smell should not be used. When in doubt, do not gamble: ask a pharmacist or return it for a replacement.
